ABSTRACT

The CRISPR (clustered regularly interspaced short palindromic repeats)-Cas (CRISPR associated proteins) system is an adaptive immune system of bacteria and archaea against phages, plasmids and other exogenous genetic materials. The system uses a special RNA (CRISPR RNA, crRNA) guided endonuclease to cut the exogenous genetic materials complementary to crRNA, thus blocking the infection of exogenous nucleic acid. According to the composition of the effector complex, CRISPR-Cas system can be divided into two categories: class 1 (including type Ⅰ, Ⅳ, and Ⅲ) and class 2 (including type Ⅱ, Ⅴ, and Ⅵ). Several CRISPR-Cas systems have been found to have very strong ability to specifically target RNA editing, such as type Ⅵ CRISPR-Cas13 system and type Ⅲ CRISPR-Cas7-11 system. Recently, several systems have been widely used in the field of RNA editing, making them a powerful tool for gene editing. Understanding the composition, structure, molecular mechanism and potential application of RNA-targeting CRISPR-Cas systems will facilitate the mechanistic research of this system and provide new ideas for developing gene editing tools.

ABSTRACT

Objective:To study the application effect of modified butterfly needle fixation method for implantable venous access port in breast cancer patients, in order to povide reference for the clinical applicaiton.Methods:A total of 300 patients with breast cancer from Janurary to December 2019 in Sun Yat-Sen University Cancer Center were divided into two groups by random digits table method: experimental group and control group, 150 cases in each group. The experimental group was treated with modified implantable intravenous port butterfly needle fixation method, while the control group was treated with conventional fixation method. The Visual Analogue Scale of pain during needle extraction, the incidence of needle stick injury and the time of needle extraction were measured between the two groups.Results:The Visual Analogue Scale of pain, the time of needle pulling were (4.01±0.89) points, (2.71±0.52) min in the control group, and (1.84±0.84) points, (1.86±0.39) min in the experimental group, the differences were statistically significant ( t values were -21.70, -16.02, P<0.01). The incidence of needle stick injury was 4.00% (6/150) in the control group and no needlestick injury event occurred in the experimental group, the difference was statistically significant (Fisher exact probability, P<0.05). Conclusions:The modified method of dressing fixation can effectively reduce the pain during needle pulling, reduce the incidence of needle stick injury, and effectively shorten the time of needle pulling.

ABSTRACT

Objective:To analyze the body composition of breast cancer patients and the changes with age; to compare the incidence of obesity in breast cancer patients with different diagnostic criteria; To understand the relationship between body mass index (BMI) and body composition; to investigate the incidence of sarcopenia and its relationship with obesity in breast cancer patients.Methods:The bioelectrical impedance technique was used to analyze the body composition of 1 187 female breast cancer patients before surgery.Results:There was a statistically significant difference between different age groups of breast cancer patients with various body composition indicators ( F values were 3.767-32.627, P < 0.01), and the incidence of obesity and sarcopenia was different in different age groups ( χ2 value was 20.819, P < 0.01). The obesity detection rate of different diagnostic methods was different. The obesity rate diagnosed by body fat percentage (PBF) was the highest. 28.14% (334/1 187) of breast cancer patients were diagnosed as "invisible obesity", which refers to normal or low BMI but excessive PBF. BMI was positively correlated with all body composition indicators ( r values were 0.137-0.954, P < 0.01), and moderately correlated with PBF and skeletal muscle mass ( r values were 0.761, 0.534, P < 0.01). The incidence of sarcopenia in breast cancer patients was 8.26% (98/1 187). 8.78% (64/1 187) of the patients with normal BMI were diagnosed as Sarcopenia. Among patients with excess PBF and excess visceral fat area, 6.70% (47/1 187) and 5.98% (15/1 187) were diagnosed with sarcopenia, respectively. Conclusion:The incidence of PBF obesity in breast cancer patients is high, and some patients have sarcopenia, which is not good for prognosis. Bioelectrical impedance technology can accurately assess the body composition of patients, and can find "invisible obesity" and sarcopenia that cannot be diagnosed by BMI, which is worthy of further promotion and application in clinical practice.

ABSTRACT

Objective:To explore the effect of dynamic multi-dimensional management model based on informatization on improving the home self-care of lymphedema patients.Methods:A total of 80 patients who received lymphedema combined detumescence treatment from January 2018 to June 2018 were selected as the control group.From July 2018 to December 2018, 80 patients were treated as the intervention group, and the whole process was managed on the basis of the control group using the dynamic multidimensional management mode based on informatization. Patients' compliance and quality of life were compared between the two groups.Results:The intervention group patients' compliance was better than control group, correctly measure the circumference of limbs, the law of life that occupy the home was good, correct manual lymph drainage direction, bandages, functional exercise, 18 standards compliance, respectively, 95.0%(76/80), 87.5%(70/80), 97.5%(78/80), 95.0%(76/80), 100.0%(80/80), 87.5%(70/80), better than that of control group were 77.5%(62/80), 75.0%(60/80), 80.0%(64/80), 70.0%(56/80), 75.0%(60/80), 50.0%(40/80), difference was statistically significant ( χ2 values were 4.103-26.182, P<0.05 or 0.01). After a course of treatment and 3 months of home-based self-care, the scores of role function, physical function, emotional function and social function of the quality of life in the intervention group were (75.44 ± 2.78), (80.11 ± 3.01), (78.25 ± 2.78), (79.93 ± 5.43), which were better than those of the control group (66.25 ± 2.58), (75.88 ± 3.57), (65.16 ± 2.47), (62.78 ± 4.01), with statistically significant differences ( t values were 8.09, 31.50, P<0.01). Conclusions:The dynamic multi-dimensional management model based on informationization and doctor-patient participation can improve the compliance and quality of life of patients participating in home-based self-care and better meet the needs of home-based nursing.

ABSTRACT

Objective:To investigate the effect of complete decongestive therapy in the treatment of severe grade Ⅲ lower limb lymphedema.Methods:From January 2018 to December 2018, The patients were admitted to the lymphedema clinic of the cancer prevention and treatment center of Sun Yat-sen University, seven patients with severe gradeⅢ lower limb lymphedema were intervened with complete decongestive therapy, including problem skin care, special techniques of unarmed lymphatic drainage, foam block bandage combined with air wave pressure therapy, filled elastic bandage pressure bandage, functional exercise combined with home bare-handed lymphatic drainage and weight loss management. The intervention had two courses, and 20 times was a course of treatment. Perimeter measurement and weight measurement were used to evaluate the therapeutic effect at the 0, 5, 10, 15, 20, 30, 40 times of treatment.Results:After two courses of treatment, the circumference value of each measurement point on the affected side was lower than that before treatment, and the difference was statistically significant ( F values were 7.449-23.073, P < 0.05). The circumference value of the affected side decreased by 7.10 - 24.53 cm, and the weight after treatment was 9.0 - 20.5 kg less than that before treatment. During the follow-up period, it was found that the lower extremity diameter value at 3 months of follow-up at 5 sites increased and tended to be stable compared with that at 1 year of follow-up. Conclusion:Step 6 comprehensive swelling treatment can obviously improve the symptoms of patients with lower limb lymphedema of this research, and in the subsequent follow-up found that self-therapy at home, no recurrence or aggravate the limb swelling degree, and can reduce or stability treatment effect, enhance confidence in the treatment of patients, so as to improve the quality of life of patients and treatment compliance.

ABSTRACT

Objective To understand students' attitude changes before and after they started their internships in the department of psychiatry and evaluate their teachers' teaching methods. Methods Using a self-made questionnaire of Psychiatric Internship Attitude that has passed the reliability and validity test, from 2015 to 2016, a total of 208 undergraduate students in the clinical medicine mental health direction of Chongqing Medical University were selected through cluster random sampling to investigate their attitude changes towards psychiatry before and after they started their internships. At the end of their internships, another self-made Psychiatric Teach Evaluation Forming Questionnaire of the reliability and validity test was used to preliminarily evaluate the students' assessment of the teaching situation of their teachers during the internship. Results Before the internships, through the survey we found that 44.2% of the students felt nervous, 37.0% fearful, 57.2% compassionate, and the percentage of students who were willing to work in the department of psychiatry is only 41.4%. Through investigation, teachers carried out the teaching work with targeted goals, at the end of the internship the tension and fear decreased to 0.0%, the proportion of empathy rose to 78.4%, and the differences are statistically significant (P<0.05). Conclusion The students' attitude survey at the beginning of their internship can help teachers improve their teaching attitudes, con-cepts and methods, and make students' negative attitudes towards mental patients and psychiatry significantly reduce.

ABSTRACT

Objective To evaluate the effect of pidotimod combined with antibacterials on the immunologic functions and clinical curative efficacy for patients undergoing recurrent respiratory tract infections.Methods Patients with recurrent respiratory tract infections (150 cases) were enrolled in Matemal and Child Health Hospital of Guangyuan from August 2013 to January 2016,which divided to two groups randomly,Group A (n =75) accepted traditional antibacterials treatment,and Group B (n =75) adopted pidotimod combined with antibacterials.The clinical symptom and immunologic functions of patients during treatment were tested,and the clinical curative efficacy of patients was evaluated in the follow-up period.Results After treatment,the disappear time of cough,fever,tonsil inflammation and pulmonary wheezing in treatment group was significantly shorter than that of control group (P < 0.05).After treatment,the levels of serum immunoglobulin in two groups were higher than those before treatment (P < 0.05),and the treatment group was significantly higher than control group (P < 0.05).After treatment,the relative activities of CD3+,CD4+,CD8+,and NK cells in two groups increased significantly (P < 0.05),the relative content of CD4+/CD8+ was also increased (P <0.05),and the improvement of above indexes in the treatment group was more obvious than those in control group (P < 0.05).After treatment,total effective rate of treatment group was significantly higher than that of control group (P < 0.05),the recurrence of infection in the treatment group is significantly lower than that in the control group (P < 0.05),and the recurrence duration was significantly shorter than control group (P < 0.05).Conclusion The clinical curative of pidotimod combined with antibacterials was remarkable for patients of recurrent respiratory tract infections,which deserved popularization not only improving the immunologic functions,but also decreasing the times of recurrence.

ABSTRACT

Objective To explore the characteristic of fractional amplitude of low frequency fluctuation (fALFF) and the relationship with the severity of depression, suicidal ideation and suicide risk in depression patients with suicidal ideation with resting-state functional magnetic resonance imaging (rs-fMRI). Methods Resting state functional magnetic resonance imaging maps were conducted using fractional amplitude of low frequency fluctuation (fALFF) in 52 depression patients (30 with suicidal ideation and 22 without) and 21 healthy controls (HCs). The severity of depression was evaluat-ed by using Hamilton Depression scale(HAMD). The suicidal ideation, the suicide risk in depression patients with sui-cidal ideation were both assessed by the Beck Scale for Suicide Ideation. The correlation between the fALFF value and the score of HAMD and the Beck Scale for Suicide Ideation was analyzed. Results MRI revealed significant differences in fALFF in the left superior/middle occipital gyrus and the right middle/inferior occipital gyrus (P<0.05, AlphaSim cor-rected)between depression patients with suicidal ideation and the HCs. Compared to the HCs, depression patients with-out suicidal ideation showed a higher fALFF in the left middle occipital gyrus (P<0.05, AlphaSim corrected). MRI re-vealed significant differences in fALFF in the left middle occipital gyrus (P<0.01, AlphaSim corrected)and the right mid-dle occipital gyrus (P<0.01, AlphaSim corrected) between depression patients with suicidal ideation and without. The fALFF of left middle occipital gyrus (r=0.366, P=0.046) and right middle occipital gyrus (r=0.513, P=0.004) were posi-tively correlated with the scores of HAMD, respectively whereas were not correlated with suicidal ideation and suicide risk. Conclusions Depression patients with suicidal ideation have an abnormal spontaneous activity in their left and right middle occipital gyrus. The increased activity in these brain areas are probably associated with the severity of de-pression whereas are not associated with suicidal ideation or suicide risk.

ABSTRACT

We investigated the baseline brain activity level in patients with major depressive disorder (MDD) by am plitude of low-frequency fluctuation (ALFF) based on resting-state functional MRI (fMRI). We examined 13 patients in the MDD group and 14 healthy volunteers in the control group by resting-state fMRI on GE Signa 3.0T. We calculated and compared the ALFF values of the two groups. In the MDD group, ALFF values in the right medial prefrontal were higher than those in control group, with statistically significant differences (P < 0.001). ALFF values in the left parietal in the MDD group were lower than those in control group with statistically significant differences (P < 0.001). This resting-state fMRI study suggested that the alteration brain activity in the right medial prefrontal and left parietal ALFF contributed to the understanding of the pathophysiological mechanism of MDD patients.

ABSTRACT

ObjectiveTo explore the status of social anxiety and study the relationship between social anxiety and self-consciousness,self-esteem among primary students.MethodsUsing stratified randomization sampling method,568 primary students in grade 4 ~ 6 were investigated with social anxiety scale for children( SASC ),self-consciousness scale for children(PHCSS) and self-esteem scale(SES).Results①The score of SASC in the group of primary students was obviously higher than that of Chinese norm (P<0.01).②The SASC and negative evaluation scores in the female were higher than the male (P < 0.05 ),the negative evaluation scores in the sixth grade (3.22 ± 2.82) was higher than the fifth grade (2.53 ± 2.76,P < 0.01 ) and fourth grade ( 2.54 ± 2.68,P< 0.05 ).The primary students in migrant workers elementary school,the single parent family and the relatively low income family had higher social anxiety scores than others(P<0.05 ).③The SASC and two major factors were significantly negative correlated with the self-consciousness and each subscale( r =-0.202 ~ -0.511,P < 0.01 ),and positive correlated with the self-esteem( r =0.282,0.270,0.305,P < 0.01 ).④)The anxiety and gregariousness had significant prediction function of 25.5% on negative evaluation,and 28.2% on SASC,anxiety,gregariousness,intelligence and school situation had significant prediction function of 20.4% on social avoidance and distress.ConclusionsPrimary students in grade 4 ~ 6 existed social anxiety.There are significant differences for their level of anxiety in different gender,grade,elementary school,family structure,monthly income.And social anxiety is closely related to self-consciousness and self-esteem.

ABSTRACT

Objective To investigate the impact of genetic and environmental factors on mental health status in adolescents twins. Methods A total of 52 pairs of twins aged 11 years were recruited with support from educational committees and school. After the guardians of these twins had signed an informed consent form,the Chinese version growth and the state of health evaluation (Development and Well-Bing Assessment,DAWBA) carries on twins' mental health growth condition investigation to the twins of Age greater than 11-year-old,and focus on a-nalysis of DAWBA youth-assessment version of more than 11-year-old twins. Buccal mucosa samples were collected from all twins for DNA extraction and zygosity identification test. Results The mood, the behavior symptom and the symptom produced the influence of the DAWBA and in the computer diagnosis' s result showed that the intra-pair correlation coefficience of the emotional disorder(rMZ=0, 200, P = 0. 28; r DZ = 0. 198, P = 0. 447) (MZ means was monozygotic,DZ means was dizygotie, r means was correlation), the special phobia symptom(r MZ = 0.440, P = 0.013;rDZ =0.419, P=0.094),the social phobia (rMZ =0.2%, P = 0. 106; r DZ = -0.119, P = 0.648),the depressive symptom (rMZ =0.556, P=0.001; rDZ = -0.254, P=0.325), and the oppositional/con-duct disorder(rMZ =0.503, P = 0.014; rDZ = -0.270, P = 0.295),eating disorder(r MZ =0.764, P=0.046; rDZ = -0.091, P=0.728) in Monozygotic twins was more remarkable than that in dizygotie twin. Conclusion The genetic factors play the vital role in affects in the child twins' mental health growth.

ABSTRACT

Objective To discuss the impact of relevant psycho-social factors on the driving adaptability. Methods A total of 870 vehicle drivers were screened out by stratified cluster sampling,and 620 drivers were finally selected for this study after eliminating those who had 1 -2 accidents. The 620 drivers were divided into accident group and non-accident group. Demographic questionnaire, life event scale, SCL-90 Scale and Eysenck Personality Questionnaire were applied to investigate their general condition, life event, mental health and personality. All data were analyzed by t test and stepwise Fisher discriminatory, and then the discriminatory equations of driving eligibility were established. Results The scores of factors including positive life events, negative life events, family events, work events and social events in accident group were 9.62 ± 11.36, 34.53 ± 30.01, 24.05 ± 23.09, 16.78 ± 17. 12 and 2.82 ± 5.07, respectively, which were higher than 4.67 ± 6.25, 6.38 ± 10. 15, 6.48 ± 9.43, 3.96 ±6.55 and 2.02 ± 5.43 respectively in the non-accident group ( P < 0. 01 ). The total SCL-90 score and the scores of all factors in the accident group were higher than those in the non-accident group ( P <0. 01 ). The Eysenck Personality Questionnaire scores of the accident group were significantly higher than the non-accident group in N dimension ( 56. 32 ± 8.77 vs. 45.23 ± 8.06 ) and P dimension (52.22 ±10.43 vs. 48.31 ± 9.35 ) ( P < 0. 01 ). Fisher discriminatory equations: Faccident group = 0. 258 × X6 + 0.119 ×X9 +0.637 ×X17 +0.043 × X5-7.476; Fnon-accident group =0. 137 × X6-1. 000 ×X9 + 1.423 ×X17 +0.003× X5-2.601. Total discrimination accuracy of verification was 88.3%, and the total accuracy of prospective discrimination was 88.0%. Conclusions There is clear correlation of driving adaptability with coercion, anxiety, emotion and negative factors. The discriminatory equations are useful for selecting drivers because of high accuracy, and may play an important role in reducing the occurrence of traffic accidents.

CONCLUSION: The results suggest that D?H gene-4784-4803del polymorphism may not be associated with qualitative and quantitative characters of schizophrenia. Male schizophrenia patients with Ins/Del encoding D?H gene-4784-4803del are significantly characterized with the character of "preoccupation".

ABSTRACT

BACKGROUND: Multiple applications of opium medicines can induce the accommodative changes of morphology and function in some intracerebral nerve positions. These accommodative changes are important neurobiological bases inducing drug-desire and re-addiction after detoxification. However, the actual molecular mechanism is unclear at present.OBJECTIVE: To investigate the impacts of the generation of heroin-dependence and detoxification on brain-derived neurotrophic factor (BDNF) in rat to provide a laboratorial gist for the participation of BDNS in heroin-dependence and detoxification.DESIGN: A randomized controlled study by employing experimental animals as subjectsSETTING: Mental health center of a medical university affiliated hospital MATERIALS: The study was conducted in the Laboratory of Pharmacology,Faculty of Pharmacology, Chongqing Medical University between March 2004and July 2004. Totally 30 inbreeding clean male SD rats with a bodymass between 200 g and 250 g were obtained from the Experimental Animal Center of the Third Military Medical University of Chinese PLA. Rats were randomly divided into blank control group(control group), heroin-dependent group (heroin group), and naloxone detoxification group(naloxone group) with 10rats each.METHODS: Morphine was subcutaneously injected into the rat with dose-increasing method to establish heroin-dependence rat model. Rats of naloxone group received subcutaneously injection of 2 mg/kg of naloxone to excite abstinent symptoms. The same dose of normal saline (NS) was injected in rats of control group. Model rats of each group were observed biologically and behaviorally. BDNF expression at different brain zone of rats in three different groups was tested with immunohistochemistry and digoxin-labeled oligonucleoide probe in situ hybridization technique.Comparison of the evaluation of abstinent symptoms in rats of each group.RESULTS: In the heroin group, the relative content of BDNF protein was higher in frontal lobe cortex, locus caeruleus and hippocampus than that of the control group( P < 0.05); BDNFmRNA relative content was higher in frontal lobe cortex than that of the control group( P < 0. 05) . In naloxone group, BDNF and its mRNA relative contents in frontal lobe cortex, locus caeruleus and hippocampus were higher than that of heroin group and control group ( P < 0.05 ).CONCLUSION: Chronic administration of heroin could affect BDNF protein and its mRNA expressions in the corresponding brain areas of the rats, which suggests that the change of BDNF expression participates in heroin-dependence and detoxification.

ABSTRACT

Objective:To develop the adolescent's family Satisfaction scale(AFSS)and test the reliability and validity of it. Methods:Obtain the structure and items on the basis of an open structure questionnaire survey and literature review,and then 1253 middle school students were tested with AFSS to test the reliability and validity of the scale.Results:The AFSS consists of 42 items and reflects three factors including family circumstance, the way of families fostering the child, communication and understanding of family. The test-retest reliability ranged from 0.882 to 0.972,and the correlations between factors and the whole questionnaire ranged from 0.899 to 0.906,which all met the criteria for adequacy of fit.Loading of the items were from 0.458 to 0.753, which explain 51.532%of the total variance.Conclusion:The AFSS is a reliable and valid scale for assessing the family satisfaction of adolescents.

ABSTRACT

Objective To evaluate the diagnostic value of Achilles tendon rupture by using X-ray plain films and MR images. Methods The plain films and MRI findings in 21 patients with operation proved acute Achilles tendon rupture were analyzed, and every manifestation on MR and operation were compared.Results The Achilles tendon rupture was diagnosed on MRI in all 21 cases. Complete rupture of Achilles tendon was showed in 7 and partial rupture in 14. MRI appearance of the rupture represented as thickening of Achilles tendon, hyperintensity in the tendon, and completely or partly discontinuance of the fibrous bundle. X-ray showed Achilles tendon rupture in 5, suspicious rupture in 12, and normal in 4. Operation showed Achilles tendon complete rupture in 9 and partial rupture in 12.Conclusion MRI can accurately show the degree and location of acute Achilles tendon rupture, which is very helpful for treatment. X-ray is the adminicular method in detecting Achilles tendon rupture and the diagnosis should be depended on MRI to confirm the the acute Achilles tendon rupture.

Objective To analyze MRI findings and its clinical significance in complications of polyamide hydrogel injection for augmentation mammoplasty. Methods The complication findings of MR imaging in 20 cases with polyamide hydrogel injection for augmentation mammoplasty were retrospectively reviewed and analyzed. Results In 20 patients, 26 breasts suffered from complications, including infection (n=5) with pieces of long T_1and long T_2 signals, aseptic inflammation (n=2) with pieces of slight long T_1and moderate T_2 signals, hard nodule (n=10) with long T_1 and long T_2 signals, and rupture (n=5) with pieces and nodes of long T_1 and long T_2 signals on MR images. Conclusion MRI has the great diagnostic value in the detection of complications after polyamide hydrogel injection for augmentation mammoplasty, and it should be taken as the first diagnostic choice.
